Temporary change to fit notes

For Statutory Sick Pay (SSP), where employees go off sick on or after 10 December 2021, employers can only ask employees for proof of sickness (such as a fit note) after 28 days of sickness (including non-working days). Proof of sickness cannot be requested earlier than 28 days.

Fit notes do not have to be provided for DWP benefit claims until 27 January. This will not affect claims to benefit.

Covid Boosters

All adults are eligible to book a COVID booster.

Appointments are available for walk-ins at Winchester Farm, Draycott Road, Cheddar BS27 3RP

To book on-line, please book herewith on the National Booking System:

You can book your booster for 3 months from the date of second vaccine, and you can access the National Booking System to book the appointment from 2 months.

The NHS is working at pace to increase the availability of appointments so that there are plenty of options for people on the National Booking System. Some local vaccination services may also contact patients directly.

New appointments are being added every day across the vaccination services. 

In terms of any planned appointments with  Axbridge & Wedmore Medical Practice, everyone should attend their planned appointments as normal. If we need to make any changes, we will contact you directly.

While most Covid restrictions ended on Monday 19 July in England, in healthcare settings like doctor’s surgeries, hospitals, dental practices, optometrists and pharmacies, people must continue to wear face coverings and follow social distancing rules. This will help to ensure the safety of our patients, visitors and staff, especially those who are more vulnerable to infection.

Seeing Patients Face to Face

We would like to reassure you that we have been seeing patients face-to-face throughout the COVID-19 pandemic.  We have never "closed" although we continue to follow national guidance to reduce unnecessary footfall into the surgery.  If you need non-urgent medical advice, please use e-Consult by clicking on the link on this home page.  Your request will be passed to our clinical team who will assess your request and, if you do need to be seen face-to-face, you will be offered an appointment to come in.

Introducing the NHS App

We are encouraging our patients to download the NHS mobile phone app. The NHS app has many features including the facility to see your available medicines, request a new repeat prescription and choose a pharmacy for your prescriptions to be sent to. You are also able to see your COVID-19 vaccination status.  As of 21 June, you will no longer be able to order repeat prescriptions over the telephone and we therefore recommend the use of the NHS app to order repeat medication in future.

Breast Screening - We have been advised that those of our patients that are eligible for breast screening will be invited in the next few weeks to contact Somerset Breast Screening Service to book an appointment.  We encourage eligible patients to make an appointment for this screening.

Long Term Condition Management

If you are unable to complete the on-line questionnaire please click on the link below to download a paper copy which you can print off and complete at home.  Please check your letter carefully as this will tell you which sections of the questionnaire you need to complete.  Most people do not have to complete all the sections so it is important to check your letter before you start.  When you have completed the questionnaire, please send it in the post to Axbridge Surgery.

Home Blood Pressure Monitoring

If you have been asked to do a seven day blood pressure reading, please download the form and follow the instructions:

  • Measure your blood pressure and pulse twice a day for seven days, morning and evening
  • For best results, sit comfortably with both feet on the floor for 10 minutes before taking the measurement
  • Each time take a minimum of two readings
  • Leave at least ONE minute between each reading and record the lowest reading
  • Record these numbers in table below
  • Return this form to the surgery once you have completed the seven day record

Improved Access

Somerset has been chosen by NHS England to introduce a new scheme that will allow you to make an appointment to see a health professional in a GP practice outside the usual working hours of GP surgeries.  The scheme works by getting a gropu of GP practices together so that they can open up in turn at the new times.

The following surgeries are offering Improved Access to all patients in the North Sedgemoor Federation.  If you cannot access an appointment at your own surgery it is now possible to access available appointments at other surgeries in your area,  please ask at Reception:

  • Brent Area Medical Centre
  • Burnham and Berrow Medical Centre
  • Cheddar Medical Centre
  • Highbridge Medical Centre
